    Whistleblower form.

    A secure reporting channel compliant with the Polish Whistleblower Protection Act. Reports are handled confidentially by a designated compliance team.

    The report is confidential and stored in a protected register.

    Data is transmitted over an encrypted connection (HTTPS), and access to the content of the report is limited to persons authorised to carry out follow-up action. Before completing the form, please review the information on the reporting process on the Whistleblowing page.

    Your report is confidential. You may submit it anonymously — in that case, a reference code will allow subsequent communication without disclosing your identity. Reports are reviewed by Zwyrtek Group's designated compliance team, within the time limits set out in the Polish Whistleblower Protection Act. Acknowledgement of receipt of the report is provided within 7 days; for anonymous reports, contact takes place solely via the reference number.

    A report may be submitted by anyone who has obtained information about a breach in a work-related context: an employee, former employee, candidate, trainee, volunteer, contractor, partner, as well as a business partner and its employees.

    Up to 3 files, 5 MB each. Allowed formats: PDF, PNG, JPG, TXT, DOCX. Files are stored in a private repository accessible only to the reporting coordinator.

Data is transmitted over an encrypted (HTTPS) connection and stored in Zwyrtek Group's protected register, without being shared with marketing systems, CRM or analytics tools.
